All events run by MLS staff and students must first receive approval and be submitted for risk assessment. No rooms are to be onsold to external parties without prior permission from the Deputy Dean or FGM.
Please submit the completed form to the Facility Manager at email@example.com or via Information Systems & Services at least 4 weeks prior to your event. A copy of this form will be held in the MLS EHS files.
